Thank you for selecting Dr. Maul’s Family Practice in Rowlett and Heath, Texas for your family’s medical care. To schedule an appointment, please call the location you would like to visit or use our online appointment request form.

Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit, along with any diagnostic films or other test results that may have been ordered or performed by another physician. Completing the forms prior to your visit will help speed up the new patient registration process.

Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

  1. Make an Appointment
  2. Sign up for our patient portal
  3. Download your patient forms online through the patient portal

When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.

To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.

Remember to bring:

  • Your insurance card
  • Valid photo ID
  • List of current medications
  • Office co-pay

In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.

For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.

Dr. Maul’s Family Practice welcomes walk-in appointments when your medical needs can’t wait. If you are sick today, we will see you today. We encourage you to make a convenient, same-day appointment if possible to expedite your treatment, but appointments are not necessary at our Rowlett and Heath, Texas locations. Our goal is to help you feel better fast.

If you are an uninsured patient, our Discounted Cash Pay Program offers reduced rates to help qualified patients obtain affordable health care. At Dr. Maul’s Family Practice, we feel that everyone is entitled to quality, affordable family health care when they need it and at fees within their means.

Feeling under the weather? Schedule a same-day urgent appointment at Dr. Maul’s Family Practice by calling the office you want to visit in Rowlett or Heath, Texas. For your convenience, we offer Saturday appointments at our Lakeview Parkway location.

Patient Forms

Authorization for Release of Medical Information (PDF) - Allows patients to authorize the disclosure of their health information to a designated individual, company, agency, or facility.

Authorization and Consent for Treatment (PDF) - All patients must provide their consent for treatment, communications (calls, emails, and text messaging), and agreement of financial responsibility.

Preferred Contacts (PDF) - Patients are encouraged to complete and return the Preferred Contacts Form but it is not required.

Office Policies

Financial Policy (PDF) - This form advises patients of their complete financial responsibility for all medical services received without regard to insurance eligibility or coverage determinations.

Notice of Privacy Practices (PDF) - Describes how health information about you (as a patient of this Care Center) may be used and disclosed, and how you can get access to your individually identifiable health information. Please review this notice carefully.

Referrals

If one of our providers suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require three business days for the processing of routine referrals.

Prescription Requests

Prescriptions and refills for established patients of Dr. Maul’s Family Practice are routinely issued during our regular office hours (see above). You can make your request by calling the office you normally visit or visiting our patient portal.

No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
